We have a permanent opportunity available for a dynamic Integrated Service Case Manager to join our New Plymouth team.  The Integrated Services Case Manager (ISCM) offers a family focussed case management approach, allowing us to look at a family’s circumstances as a whole and to work with them to reduce their level of risk, and build on their strengths.

 

An important part of the role is working with families with high and complex needs under the Integrated Service Response.  The Integrated Services Response component will require a strong focus on inter agency collaboration with Oranga Tamariki, Housing New Zealand, New Zealand Police, Integrity Services and Partnerships and Programmes as well other key Government agencies, Non-Government Organisations and local community to construct comprehensive plans to support clients’ needs.  The overall goal is to provide wrap around support to enable clients to gain independence and move into employment.

 

The other equally important component of Integrated Services Case Management is working with young people aged 18-24, providing a strong focus on assisting them to transition to independence and employment through intensive one to one support.  

 

You will achieve this by:

 

  • Regularly meeting clients, creating plans to achieve independence and employment
  • Strong inter-agency collaboration
  • Ensuring your clients are receiving full and correct entitlement and support required

 

To be successful you need:

 

  • Strong networking and relationship skills – you need to build effective partnerships with internal and external stakeholders
  • Exceptional client engagement skills
  • Sound judgement and decision-making abilities
  • Good insight and understanding of labour markets and employment opportunities
  • Ability to work constructively with youth clients and influence positive outcomes and direction
  • Strong employment focus with proven ability to achieve outcomes
  • Minimum of a restricted driver’s license
  • Ability to work in New Zealand

Remuneration for this role is from $55,821.00 to $76754.00 (Range E).  Application along this salary scale is dependent on relevant work experience, evidenced skills and other factors.
