The Team: What we do

Our North East Community Team within Older Person’s Health and Rehabilitation is a large interdisciplinary team working with clients in the community to understand support services or rehabilitation needs, with a goal of maintaining patient independence. We value our restorative model of care and our work is focused on listening to our clients, helping to support and enable people to remain independent in their homes in the community or support their journey into residential care.

We’re currently seeking an NZ Registered Social Worker to join us in this permanent part-time opportunity based at The Princess Margaret Hospital. This will see you working 28hrs per week (0.7FTE), with days and hours to be negotiated with the successful candidate.

The Role: Where you fit in

In this role, you will be responsible for the following:

  • Assessment, monitoring and support of clients within the service in accordance with ANZASW Code of Ethics and Standards of Practice
  • Providing appropriate resource and education to clients and their family/whanau
  • Participating in service development and quality assurance initiatives / processes

Your skills will include the following:

  • Excellent interpersonal skills with the ability to build and nurture relationships with clients and their family/whānau
  • The ability to work collaboratively within an interdisciplinary team
  • Experience working in older persons health and/or in a community setting is advantageous
  • Current NZ Registration with the Social Workers Board and Annual Practicing Certificate (or eligibility to obtain) is essential
