The Team: What we do

Te Whare Hohou Roko is a regional 9 bed 24 hour extended rehab forensic SMHS inpatient unit based at Hillmorton Hospital that interfaces with Department of Corrections.

The multi-disciplinary team (MDT) strives to provide a therapeutic environment in which the consumer is at the centre of care. Our team promotes a recovery and strengths base approach to facilitate individualised care. We work alongside consumers and their family/whanau to achieve their goals and safe reintegration back into the community.

We are currently seeking a Registered Social Worker to join our team on a part-time, permanent basis working 16 hours per week. The required hours of work for this role are negotiable, to be worked across two to four days.

The Role: Where you fit in

In this role, you will be responsible for the following:

  • Working alongside the MDT from admission to treatment planning, reviews and discharge planning
  • Contributing to the MDT caseload
  • Providing advocacy with other community and government agencies
  • Working collaboratively with external agencies striving for better health outcomes for our consumers
  • Working with consumers from diverse social and cultural backgrounds and working collaboratively with whanau, the wider multidisciplinary team, and other relevant agencies to ensure the best outcomes
  • Identifying and contributing towards service improvement initiatives

Your skills will include the following:

  • The ability to work both autonomously and as a part of an MDT, while simultaneously providing a voice for Social Work within the MDT
  • The ability to build and develop trusted relationships with consumers, their family/whanau, the MDT and other relevant agencies, along with a working knowledge of family/whanau support services
  • Applicants will need to be a New Zealand registered Social Worker with current Annual Practising Certificate (or eligibility to obtain)
  • A broad knowledge of legislation that ensures patient rights are maintained
  • Experience within mental health is essential and recent experience within a forensic mental health setting is desirable
  • As there is some travel required in this position with the need to take passengers, a full NZ driver’s licence is essential
