Job Description
Job Summary:Works as part of a multidisciplinary Assertive Community Treatment (ACT) team providing comprehensive, community-based services to adults living with severe and persistent mental illness. Supports clients in achieving recovery goals, maintaining housing stability, reducing hospitalizations, and improving overall quality of life. The majority of work is performed in the community, meeting clients where they live, work, and engage in daily activities to provide individualized support, care coordination, advocacy, and resource connection.
