West Farm Surgery is a small, friendly practice based in North Tyneside. Due to a restructure of our clinical team we are looking for an advanced clinical practitioner to join us. You will be responsible for managing and providing care for the presenting patient with undifferentiated, undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and longterm conditions in line with existing practice and national policies and guidelines.

The post is for 25 hours per week and we can have some flexibility regarding the days/hours worked.

Main duties of the job

To see patients independently, assess, diagnose and treat patients that request same day clinical access, using skills in history taking, problem solving and clinical decision making to establish a diagnosis and management/treatment plan
