DESCRIPTION


Part of the Care Village family West Roxbury Health & Rehabilitation Center is a 65 bed skilled nursing facility located on Washington street neighborhood of West Roxbury, MA. We are currently hiring for an Assessment Coordinator.

Formerly known as the MMQ Nurse the Assessment Coordinator works directly with the clinical team to maximize facility reimbursement. The resposbilities include but are not limited to:

Assessment Coordinators generate UDA’s Prior to MDS ARD (with specific emphasis on GG and monitor for timely completion)

o Ensure all UDA’s (nursing assessments) are filled out and locked

o Ensure all UDA’s are done timely

· UDA’s to be completed:

o BIMS (Brief Assessment for Mental Status, interview typically done by Social Services). It is to be open within 7 days leading up to MDS ARD
