About the role

As an Assessment Support Coordinator, you will be working alongside Social Workers, Occupational Therapists, and health colleagues across a range of settings to support people to remain as independent as possible and linked in with their local networks.

  • You will be undertaking strengths-based assessments and support planning, in accordance with the Care Act 2014.
  • You will assist individuals to identify their goals and how best to achieve them to support their independence, and where appropriate complete mental capacity assessments.
  • You will be working as part of a busy team and alongside other teams within the local authority. Multi-disciplinary practice will be integral to the role.
