We currently have Band 6 Care Coordinator opportunities across our Adult Community Mental Health Teams (CMHT) across the Bradford and Craven District. 

You must be either a nurse, social worker or occupational therapist with excellent interpersonal and organisational skills.

As part of the role you will be expected to manage a case load , be committed to team working and deliver skilled interventions aimed at promoting recovery, to people who have enduring, moderate to severe psychotic or affective symptoms with unstable, chaotic lifestyles. Many of our service users also have coexisting substance misuse.

Experience in working with multi-agency staff is essential as are excellent communication skills and a good working knowledge of the Care Programme Approach and Mental Health Act/Mental Capacity Act.
