We have an exciting opportunity for a Care Coordinator to join our domiciliary and extra care services across Ellesmere Port and Chester.

As a Care Coordinator , you'll play a vital role in ensuring the smooth delivery of high-quality, person-centred care. You'll work closely with care staff, the people we support, families and healthcare professionals to ensure services are safe, effective and tailored to individual needs.
