Job Description
To provide basic care coordination, discharge planning, monitor/evaluate and education for clients and their families on Day Care Service received as well as Administrative support to Day Care Utilisation and Learning and Development.
You will need to:
Information and Referral (I & R)
Manage referral sources, including emails, phone calls, and walk-ins, by evaluating client needs and matching them with appropriate services.Coordinate the referral process with clients and external stakeholders, ensuring successful service referrals and optimizing lead conversions.
