As a Care Transition Liaison, you will act as a resource for patients referred to home health or hospice, facilitating their transition between the referring physician or hospital and the LHC agency. You will educate patients and families on home health practices, ensuring the agency can meet their needs and support the Care Transition Coordinator or Account Executive within the facility.
