Join our team as a Case Manager with the Community Housing Assistance Program (CHAP) and help individuals experiencing homelessness access resources, stabilize their lives, and move toward permanent housing.

Key Responsibilities:

+ Build supportive relationships with participants experiencing homelessness.

+ Provide supportive counseling and daily living skills training.

+ Connect participants to community resources and assist with discharge planning.
