Position Overview

We are seeking a compassionate and organized Case Manager to support individuals and families in achieving stability and independence. This contract (temp-to-perm) opportunity is ideal for professionals with experience working with vulnerable populations, particularly individuals experiencing serious and persistent mental illness (SPMI).

The Case Manager will conduct assessments, develop service plans, connect clients to community resources, and monitor progress toward personal goals.
