Our Advanced Endoscopy Medical Home model of care provides an integrated patient-centered population health approach to management and treatment of Interventional Endoscopy patients, including patients undergoing diagnosis and treatment for esophageal and gastric cancer and esophageal disorders with a team including gastroenterologists, nurse practitioners, nurse coordinators, and a dietitian.
