Responsibilities

• Assists patient care in defined units by planning, managing, monitoring and advising specialist pharmaceutical care programs for patients.
• Reviews medication orders and patient medication profiles for appropriateness of therapy.
• Identifies and undertakes a medication review / history of newly admitted patients to resolve issues that may influence the patient’s treatment plan.
• Provides assistance with the treatment plans for inpatients, considering their presenting symptoms, medical history, and the evidence base for pharmacological approaches in that condition.
• Conducts clinical evaluations to assess medication use appropriateness.
• Creates future treatment options with patients /caregivers after considering and discussing the risks and benefits.
• Conducts clinical and cost effectiveness review of medications for addition / deletion from the formulary.
