Job Summary:
The Clinical Practice Coordinator (CPC) facilitates coordination between multidisciplinary teams caring for patients within their designated service-line areas to ensure high-quality outcomes on the assigned units. The CPC will collaborate with local and national experts to drive innovative care, lead organizational and system change, and optimize resources to improve patient outcomes. They will provide clinical expertise, staff education, and lead and/or assist with system-wide initiatives. The CPC demonstrates comprehensive quality nursing care and will assist the medicine service team to deliver best practice/high-quality care, meet regulatory standards, certification, and accreditation.
