This is a rare and exciting opportunity to join our existing Clinical Specialist PCN Occupational Therapist and lead the way in the proactive care of those living with frailty and dementia in Pendle West. You will join Pendle West Primary Care MDT including ARRS Paramedic ARRS, ARRS GP’s, social prescribing link workers and the Integrated Neighbourhood Team.
