We support patients and their families in the most difficult and sensitive times of their lives, offering compassion, comfort, experience and a true desire to make a meaningful difference. We are seeking dedicated and compassionate Community Care Assistants to join our day team in providing high quality care. The ideal candidates will play a vital role in supporting individuals wishing to remain at home within the community at the end of their lives. This position requires effective communication skills and the ability to work collaboratively with care teams and other care professionals to ensure the well‑being of our patients.
