We are seeking a dedicated and compassionate Community Learning Disabilities Nurse to join our multidisciplinary team. Responsibilities
- Provide specialist nursing care and health facilitation for individuals with learning disabilities.
- Undertake comprehensive health assessments and develop person‑centred care plans.
- Work collaboratively with families, carers, and other professionals across health and social care.
- Deliver health promotion, education, and training to service users, carers, and staff.
- Provide specialist support, advice and guidance to individuals, families and carers.
- Provide specialist support as part of the multi‑disciplinary team.
- Complete specialist assessments as clinically indicated.
