Superior Ambulance Service is participating in a grant-based program amongst 80+ Emergency Departments Nationwide. The Community Paramedic-Led Transitions Intervention (CPTI) team will dedicate half of their role by identify dementia patients to participate in a hospital to home transitions model of care and the other half support transport calls on the ambulance.
