The Social Worker is responsible for assessing and providing direct Social Work services and interventions to patients and families in the hospital setting upon admission and throughout discharge.

Provide ongoing case management support to the patients.
Assesses patient's needs and develops discharge care plans.
Communicate effectively with staff, colleagues, patients, and family members.
