What You'll Do

• Provide full-spectrum outpatient primary care for adults and adolescents: wellness, chronic disease management (HTN, DM, HLD, COPD/asthma), women's health basics, behavioral health integration, and urgent-care-level complaints.
• Close care gaps and optimize quality measures (preventive screenings, immunizations, chronic care targets) with strong MA/NP and central care-coordination support.
