- Replacement opening with a mix of inherited panel + new patients
- Team: 6 physicians + 1 APC
- Hours: 8:00 AM – 5:00 PM
- Clinical Environment
- Specialties: Family Medicine + Integrative services (chiropractic, acupuncture, massage)
- Facilities: No on-site lab or imaging (labs sent out; MAs do phlebotomy)
- Workspace: Shared office, 2 exam rooms per provider, procedures allowed (no dedicated procedure room)
- Support Structure
- Dedicated MA per provider
- Nursing: 2 FT RNs + 1 PT RN (triage, refills, follow-ups, MWVs)
