We are offering an exciting opportunity to join our Home First (Discharge to Assess - D2A) Service as a Shift Lead based in Leek, Staffordshire. This is an excellent development opportunity for individuals wishing to take on first line management responsibilities.
The service supports hospital discharge to assess and re‑able in the home and you will support the delivery of the service in line with service demand. Due to the nature of this service you would be required to support the service 7 days a week between 7am-10pm on a rota basis, which includes weekends and bank holidays.
