Job Title: Palliative Care Nurse Practitioner (NP)
Position Summary:
We are seeking a compassionate Palliative Care Nurse Practitioner to provide patient-centered care for individuals with serious, chronic, or life-limiting illnesses. The NP will manage symptoms, support complex decision-making, and coordinate care in collaboration with interdisciplinary teams to improve quality of life for patients and their families.
Key Responsibilities:
Conduct comprehensive assessments and manage patients with complex chronic or life-limiting illnesses.
Provide symptom management for pain, dyspnea, nausea, fatigue, anxiety, and other distressing symptoms.
Facilitate goals-of-care discussions and advance care planning, including end-of-life decisions.
Collaborate with physicians, nurses, social workers, and other team members to coordinate patient care.
