PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.
Position Summary
The Licensed Practical Nurse (LPN) provides direct patient care in accordance with applicable laws and organizational policies. The LPN supports the delivery of compassionate, high-quality care in hospice and palliative care settings, performing assigned clinical tasks and collaborating with interdisciplinary teams to ensure optimal outcomes for patients and caregivers.
Primary Location: Community Hospice UF Health located on 8th street, Jacksonville, FL 32209. Must be able to flex locations as needed.
Shift: 7 am - 7 pm/Days three twelve-hour shifts
