Job Description
Job Summary: The LVN-PACE Program (Program for All-inclusive Care for the Elderly) works under the supervision of a Registered Nurse (RN) and provides developmentally appropriate and age-specific direct and indirect patient care for participants in the adult care program which serves adults 55 years of age or older with one or more chronic or post-acute medical, cognitive, or mental health conditions for whom a qualified healthcare provider has prescribed or requested services. Provides care in accordance with the established individualized plan of care for each participant based on diagnosis, orders, medical condition, psychosocial needs, physical condition, and other condition(s) pertinent to participant's specific situation. Additionally, this position will provide care in the place of the participant's residence to evaluate home care needs, available support, and compliance with the developed Plan of Care.
