The East Riding OT Service is seeking a proactive, person centred Occupational Therapist to support adults living across our communities to remain safe, independent and well.

Responsibilities

• Carry out functional assessments, provide early intervention, and develop tailored, strengths based plans that promote independence and reduce risk.
• Contribute to front door triage, offer timely advice, and recommend equipment, rehabilitation programmes, and minor or major adaptations that enable people to live confidently at home.
• Work across a variety of community settings, including prisons, and support individuals to maintain autonomy through informed decision making.
• Collaborate closely with health, social care, housing and voluntary partners, sharing information effectively and contributing to joint assessments and MDT discussions.
