**Pay:** $52.00 - $55.90 per hour based on experience
**Type:** Three-month contract with opportunity for extension or full time offer
**Schedule:** 5 days a week, 8:00AM – 4:30PM
**Notes:** Shelby, Fayette, Tipton counties
**Primary Responsibilities:**
· OT starts of care
· Evaluations
· Recertifications
· Discharges and treatments in the community and assisted living facilities.
**Qualifications:**
· OT license in TN
· Home Health experience / Home Care Home Base experience
· Experience with cognitive therapy
