Liberty Healthcare is now hiring for a full-time Options Counselor position.
As an Options Counselor, you will actively contribute to a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities.
Service objectives include:
+ Functioning as the “go to” resource for individuals who are seeking information on long term care and other community-based services and supports
+ Providing person-centered planning to individuals in a way that supports their informed decision making and ability to secure services which will enable them to live as independently as possible
Your day-to-day responsibilities as an Options Counselor will involve:
+ Fielding a high volume of phone calls and email requests
+ Talking with individuals and their families about their needs
+ Providing information and answering questions
