About the Role: We are seeking a Community Pharmacist to join our team. This role supports patients by improving medication safety during hospital admission, discharge, and in daily life.

Responsibilities:

• Work in the outpatient pharmacy and collaborate with care teams to fulfill hospital discharge orders.


• Balance hospital discharge needs with the needs of community patients.


• Rotate as a Medication Reconciliation Pharmacist with other pharmacy staff.


• Collaborate closely with inpatient and outpatient pharmacy teams and healthcare providers.

Work Schedule:

• Monday Friday: 7:00 AM 5:30 PM.


• Saturday: 8:00 AM 4:00 PM.


• Closed Sundays.


• Rotating holidays and Saturdays.
