Description:
The candidate should be a practicing Physical Therapist in the state of Ohio with advanced training, experience, and skills treating a wide variety of Pelvic Floor conditions in both male and female patients.
Position Purpose:
- The Physical Therapist provides evaluation and treatment of patients referred for physical therapy services. The Physical Therapist communicates and collaborates with other caregivers to provide quality care and maintains accurate patient records.
What You Will Do:
- Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the manager, clinical coordinator or designee.
- Documents accurately PT evaluation results, treatment interventions, patient progress toward goals, and discharge records.
