Physical Therapist (PT)

Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We're committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

Full-Time Hours: Monday - Friday (40 Hours/Week)

Annual Salary: $80,000 – $110,000 (Based on a Per Visit Rate)

Coverage Location: New Albany, Whitehall, Gahanna, North Linden

Travel: Required + Milage Reimbursement ($0.48/Mile)

Responsibilities:

• Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Assess and evaluate therapeutic, rehabilitative/functional status.
