Easterseals UCP is excited to announce the start of a new primary care service to be part of an integrated care program with our Behavioral Health and Intellectual and Developmental Disabilities (IDD) service lines. In response to the transformation of North Carolinas Medicaid program to managed care as well as the industry shift toward value-based reimbursement and whole-person care, Easterseals UCP is developing an integrated care model across the state, consisting of primary care and behavioral health providers, care managers, and IDD specialists. The program will be modeled after research on integrated care conducted by the AIMS Center at the University of Washington. Substantial research has shown that integrated care can lead to substantial improvements in clinical outcomes and cost reductions for persons with co-occurring chronic physical and behavioral health conditions.
