Arizona state medical license with current state controlled substance registration and federal D

Outpatient drug abuse treatment, counseling and rehab facility

EA with X Waiver for Suboxone and Subutex certificate

Responsible for all medical aspects, opioid replacement therapy (maintenance and detoxification) programs (methadone), and out-patient services.

AZ medical doctor license and preferable experience with the substance use addiction and mental health.
