Position Overview

We are seeking an experienced Registered Nurse Case Manager to provide high quality home health services to patients across the Trumbull and Stamford areas. This role focuses on case management, skilled nursing care, and comprehensive patient assessments while ensuring compliance with CMS, state, and agency guidelines.

Schedule & Work Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM


• Weekly Hour Guarantee: 40 hours


• Productivity Expectation: 30 points per week


• Coverage Area: Trumbull and Stamford, with visit days clustered for efficiency


• On Call/Weekend Requirement: None


• Mileage Reimbursement: From second patient through last patient


• Pay Rate: $51 to $53 per hour
