We are looking for a compassionate and skilled Senior Mental Health Primary Care Practitioner to join our dynamic multidisciplinary team. This role offers the opportunity to make a meaningful difference by supporting adults with mental health needs directly within primary care settings, helping people access timely, person centred support closer to home.
Working as part of our Primary Care Network, you will use your clinical expertise to triage referrals, complete comprehensive assessments, and deliver brief therapeutic interventions.
