The post‑holder will provide high‑quality, patient‑centred nursing care within the Discharge Lounge, supporting safe, timely, and effective discharge processes. They will work collaboratively with the multidisciplinary team to ensure patients are clinically optimised for discharge, receive appropriate information, and experience a smooth .
This role is ideal for a nurse who enjoys fast‑paced work, problem‑solving, and supporting patients at a key transition point in their care transition from hospital to home or onward care.
