Key Responsibilities:


+ Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance.

+ Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care.

+ Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions.

+ Collect, assess, and evaluate patient health data to develop and implement care plans.

+ Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols.

+ Utilize EMR and other systems to document care accurately and efficiently.

+ Participate in shared governance and contribute to process improvements focused on patient safety, quality, and efficiency.
