Summary The Cardiac Medicine Registered Nurse Coordinator provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. This is an Open Continuous Announcement and will remain open until September 30,2026. Qualified applicants will be considered and referred as vacancies become available. Announcement may be closed earlier if no longer needed. Responsibilities Duties include but are not limited to: Triaging Cardiology Consults to determine appropriate access. Working with patients and providers to determine plan of care and treatment strategy. Providing cardiology specific information / education / instruction. Evaluating care and outcomes to ensure timely and appropriate provision of services. Establishing a therapeutic relationship, allowing the patient to attain, maintain or regain optimal function through assessment and treatment.
