About the Role

We have an exciting opportunity for a Registered Nurse with Mental Health experience to join our team of case managers within our Tauranga based Mental Health Services for Older People Community team.

This role/team is dedicated to providing high-quality care to the growing population in the Bay of Plenty. You will collaborate with referrers, healthcare partners, referred clients, their families/whānau, and the wider community to deliver efficient triage and assessments. Your goal is to facilitate and implement tailored treatment plans that promote health and independence within the community.

• Offering advice and support
• Managing and resolving mental health crises
