Employed by WellFort Community Health Services, and embedded within a partner primary care clinic in Malton, Brampton, or Rexdale, this role is an integral member of an interdisciplinary primary healthcare team. The position advances WellFort's commitment to community health, health equity, and comprehensive primary care while supporting partner clinics across the Central West Ontario Health Team.
Guided by WellFort's model of care, the successful candidate will contribute to the delivery of accessible, high-quality services focused on primary care, health promotion, disease prevention, chronic disease management, immunizations, and cancer screening.
