As a pivotal part of the Social Medicine Program, you will deliver nursing care that includes chronic disease management, harm reduction, and wound care across varied environments. Key Responsibilities:
• Deliver episodic care in non-traditional settings
• Assess and monitor patients independently for complex needs
• Provide health education and navigation support
• Identify and advocate against barriers to care
• Participate in team huddles for collaborative care planning
Requirements:
• Registration in good standing with CNO as a Registered Nurse
• 3+ years of relevant clinical experience
