Job Summary:

• The Registered Nurse is responsible for the delivery of quality patient care services.
• Responsible for establishing a Plan of Treatment individualized for each patient and supervise the staff delivering the care.
• You will build relationships with patients and families as you visit them within the comfort of their own homes.
• Conduct Comprehensive Assessments (using the OASIS tool), develop and implement Plan of Treatment
• Collaborate with physicians, agency staff and contract services to coordinate patient care
• Initiate appropriate referrals for other medical services when indicated
• Perform various types of skilled nursing visits
• Promote and maintain patient health through teaching and learning appropriate self-care techniques
• Document direct patient services in a timely manner in electronic medical records
