**Per Diem RN HOSPICE - Care Manager for Nursing Homes in the Capital Region. Come experience the rewards of working in end-of-life care!**

The Community Hospice has a wonderful opportunity for a per diem RN to work as part of our team caring for patients and families in local nursing homes in the capital region (Albany, Schenectady, Troy, etc).

Availability **needed** for this per diem position would be **during the week (Mon-Fri).** Hours generally needed are 8a-4:30p. No weekends needed at this time.
