**_Position Purpose:_** **The goal of the Acute Care Manager is to coordinate and manage care to assist patients in meeting their multiple needs, ensure handover and continuity of services, and to decrease fragmentation of care and services. Works collaboratively with members of the**
**interprofessional acute care, ambulatory and post-acute care management teams to improve communication across the continuum of care.**
