16208BRDepartment Name:HSP Care Coordination Location:Meriter Hospital Job Description: The RN Case Manager (CM) collaborates with the interdisciplinary care team in the development, use, and maintenance of a care plan and facilitates an interdisciplinary approach to patient care and discharge planning. The RN Case Manager collaborates and maintains close partnership with the Medical Social Worker and other Care Coordination team members. The RN Case Manager identifies patients with discharge needs and facilitates continuity of patient care in the transition from the hospital to the community. The RN CM collaborates with the Enterprise Utilization Management (UM) RNs and Coordinators to communicate the plan for utilization of health care resources.
