RN Case Manager

Suncoast Hospice, a part of Empath Health, is seeking a caring and motivated RN Case Manager to work in Pinellas county while providing exceptional care on the following schedule: Monday-Friday 8-5.

What you'll do:

• Under the direction of a licensed physician, manage discomfort and provide symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provide initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs and activities of daily living.
