Pay Range: $140,000 – $160,000 per year (DOE) Alternative Pay Option: $130,000 – $150,000 per year + Company Vehicle
Schedule: Monday–Friday, Full-Time
Coverage Area: Santa Clara County with occasional coverage in Alameda County
We are seeking an experienced and patient-focused RN Case Manager to join a 5-star, clinician-led home health organization serving Santa Clara and Alameda Counties.
