Job Description:
Home Health RN
**Must have at least 12 months of home health experience within the past 3 years.
Hands-On HCHB and OASIS experience required.
RN: Monday - Friday, 40 hours per week. Must meet minimum productivity expectation of 30 points per week.
Some on call is preferred.
Area of coverage is Green Bay and outlying communities.
